Shira Shavit, M.D.
It is in these efforts to make lasting change to the massive system of incarceration, with its entrenched politics, that I know Dr. Shavit best. She accelerated the conversation about transitional care during the overhaul of medical services under the current federal receivership, single-handedly creating the only discharge planning clinic inside the California state prisons to connect patients with chronic disease back to their communities.

Leader's Biography 2010:
Working in a prison as a young family physician, Shira Shavit found that many inmates with HIV and hepatitis were being released without health information or a care plan. As director of Transitions Clinic, she has led a community-based effort to create a medical home tailored to the needs of former prisoners and their families. As a member of San Francisco’s Reentry Council, Shavit works with city leaders on policies to improve the health of people returning from prison. She also designed the curriculum for the first national certificate program to train former inmates to become community health workers.
