Leaders The community health leaders are an extraordinary group of creative, health care service providers, health advocates and authentic leaders whose programs often serve as national models for building healthier communities.
The leaders come from 47 states, Washington, D.C. and Puerto Rico, from large cities and rural towns.
All represent a powerful commitment to changing the health care system by providing access to care and new ways of thinking about access in areas of great need. All are responsible for changing the health status of entire communities because of their leadership.
